To create an annual report, you need a reference number that can be obtained from the immigration account which can be accessed through this website: https://e-services.immigration.gov.ph/Auth/Login. To do this, you must log in to the website. If you're logging in for the first time, you'll need to sign up for an account.
Once you have logged in, you will be able to view your account details. Check and update any necessary information before proceeding to provide your passport details. If you have renewed your passport, make sure to update the new number until the end. Once you have completed all the required details, you will receive a unique reference number. Take a picture of it and then proceed to join the annual report queue.
The office is located on the 5th floor of Circuit Mall Ayala.
They don't accept "voluntary" ACR I-Card application here. You need to go to the main Bureau of Immigration office in Intramuros (Magallanes Drive)

The location is on the 5th floor of the Ayala Circuit Mall. Just take the elevator as if you are going to the Cinema. I got there at 8am to get my Visa extended for another month because I need a few more days in the Philippines and my current extension expires in 2 days. The process was completed by 8: 20am. It probably would have been faster but I had to go to the ATM and return. The staff were all extremely welcoming and helpful. The office was super clean and the air-conditioner was great. Once I got to the counter to turn in my paperwork the staff even patiently helped me correct the mistakes i made filling the forms out. I have already had the 29 day extension and this extension cost roughly 7700 PHP because I am required to get the ACR Card since I will be staying more than 59 days in the Philippines. Now 1500 of that was the fee for having the process expedited. Plus, it takes roughly 7 days to create the ACR Card and you will have to return to that immigration office to pick it up in person. Just figured I would pass that last bit of info about cost and ACR Card to everyone else. I was a bit shocked when they explained it since I only need an extra 5 days in country but rules are rules. Oh and the ACR Card is valid for a year. Hope this helps someone and happy travels.
